Educational Service

As part of providing an educational service to you the Educational Consultant will need to collect and record personal information that is relevant to your current situation. This information will be a necessary part of the educational assessment/ tutoring that is conducted. You do not have to give all your personal information, but it is helpful for the Educational Consultant to have a broad understanding about you.

Purpose of collecting and holding Information.

The information is gathered as part of assessment and assists in planning programs, Individual Education Plans and tuition sessions. This is only seen by the consultant. The information is retained by the consultant and enables the consultant to provide a relevant and informed educational service.

Access to client information

At any stage you as a client are entitled to access the information kept on file.

Confidentiality

All personal information gathered by the educational consultant during the provision of the service will remain confidential and secure except where:

It is subpoenaed by a court, or

  • Failure to disclose the information would place you or another person at risk. or
  • Your prior approval has been obtained to provide a written report to another professional agency, i.e., a GP, Occupational Therapist or Speech Therapist; or discuss material with another person, e.g., a parent
  • If disclosure is otherwise required or authorised by law.

Fees

Initial consultations are usually 50-60 minutes. An initial consultation fee is $240, for assessments the fees are $195 per session, which is payable at the end of each session by cash, credit card or EFTPOS. There is a $300 report fee payable once the report has been completed.

If you have any recent school reports or any other reports from allied health professionals, please send them to estelle@switchedonconsultingservices.com.au

Cancellation policy

If for some reason you need to change or cancel an appointment, please give 24 hours’ notice. Failing to do so will incur a cancellation fee.
